Konjestif Kalp Yetmezliği, Hipertansiyon, Tip 2 Diyabet, Kardiyomegali ve Akromegalisi Olan Hastanın Gordon’un Fonksiyonel Sağlık Örüntüleri Modeli ve Nanda Hemşirelik Tanılarına Göre Hemşirelik Bakımı: Olgu Sunumu

Abstract

Yaşamı tehdit eden, hastane ortamında bakım ve tedavi gerektiren, mortalite ve morbidite artışına neden olan konjestif kalp yetmezliği (KKY) ile pankreasın ürettiği insülinin vücut tarafından etkili bir şekilde kullanılamaması ya da insülin sekresyonundaki bozukluk ile karakterize, hiperglisemi ile seyreden Tip 2 diyabet, patofizyolojik olarak birbiri ile bağlantılı kronik hastalıklardır. Tip 2 diyabet ve hipertansiyon da ateroskleroz kaynaklı kardiyovasküler hastalıkların toplum içinde yayılmasına neden olan birbiriyle ilişkili hastalıklardır. Yine kalbin hem dilatasyonu hem hipertrofisi olan kardiyomegali ile büyüme hormonu fazlalığından kaynaklanan akromegali klinik bir sendrom olarak birbirleri ile bağlantılıdır. Birbiri ile ilişkili komplike tanıları olan hastalarda yaşam kalitesinin arttırılması ve komplikasyonların önlenmesi için standardize edilmiş hemşirelik bakımının uygulanması gereklidir. Bu olgu sunumunda KKY, hipertansiyon, Tip 2 diyabet, kardiyomegali ve akromegali tanıları olan bir hastanın hemşirelik bakımı, Gordon’un Fonksiyonel Sağlık Örüntüleri (FSÖ) Modeli ve Kuzey Amerikan Hemşirelik Tanıları Birliği (NANDA) hemşirelik tanıları kullanılarak sunulmuştur. Olgunun verileri, hasta bakımı sırasında yapılan gözlemler ve hasta ile yapılan görüşmelerden elde edilerek toplanmış, modele göre bakım içeriği planlanarak uygulanmış ve elde edilen sonuçlar değerlendirilmiştir. Hastaya sunulan holistik hemşirelik bakımı ile olumlu sonuçlar elde edilmiştir.

Life-threatening, require care and treatment in a hospital environment, the leading cause of mortality and morbidity cause of congestive heart failure (CHF) with the insulin the pancreas produces insulin secretion impairment, or inability to use efficiently by the body, which is characterized by hyperglycemia of Type 2 diabetes are chronic diseases that are connected each other as pathophysiological. Type 2 diabetes and hypertension are also interrelated diseases that lead to the spread of cardiovascular diseases caused by atherosclerosis in society. Yet, cardiomegaly is both dilatation and hypertrophy of the heart, and acromegaly caused by an excess of growth hormone, are related to each other as a clinical syndrome. In order to improve the quality of life and prevent complications in patients with complicated diagnoses associated with each other, it is necessary to apply standardized nursing care. In this case report, the nursing care of congestive heart failure, hypertension, Type 2 diabetes, cardiomegaly, acromegaly patient is presented using the Functional Health Patterns Model (FHP) and the North American Association of Nursing Diagnostics (NANDA) nursing diagnoses. The data of the case was collected from the observations made during patient care and interviews with the patient, the care content was planned and applied according to the model and the results were evaluated. Positive results were obtained with the holistic nursing care offered to the patient.
