Diyabet ve Kanser Riski Arasındaki İlişki: Bir Şemsiye Derlemesi

Abstract

Amaç: Dünya çapında diyabet ve kanser insidans oranlarının giderek artması nedeniyle bu alanda yapılan çalışmalardan elde edilen verilerin güncellemesi, kapsamlı ve erişilebilir bir özetine gereksinim mevcuttur. Bu araştırmada, diyabet ve kanser riski arasındaki ilişkiyi değerlendiren literatürün sistematik bir incelemesinin güncellenmesi ve özetlenmesi amaçlandı.

Gereç ve Yöntem: Bu çalışmada, Joanna Briggs Enstitüsü (Joanna Briggs Institution) tarafından hazırlanmış mevcut sistematik derlemelerin ve meta analizlerin kanıt sentezleri için tasarlanan “şemsiye derleme" tasarımı kullanıldı. Şemsiye derleme metodolojisi doğrultusunda çalışma kanıtları özeti derlenerek, yaygınlık oranları (yüzde, oran, göreceli risk- RR) ve heterojenite oranları incelenerek nicel bir raporlama yapıldı.

Bulgular: Şemsiye derlemeye dahil edilen çalışmalarda, diyabet ve kanser riski arasında bir ilişki olduğu gösterildi. Yapılan metodolojik kalite değerlendirmesinde, dahil edilen çalışmaların tamamının bir yönerge doğrultusunda yapılandırıldığı, ancak araştırma tasarımlarının PICO tabanında oluşturulmadığı saptandı. 

Sonuç: Sonuç olarak, diyabetin kolorektal, pankreas, endometriyal ve over kanser gelişimi için önemli bir risk faktörü olduğu saptandı. Bu şemsiye derlemede, diyabet ile ilişki olarak farklı kanser türleri ve farklı popülasyonlarda daha fazla araştırma yapılması, kanıtları sentezlemek ve özetlemek için standart prosedürlerin izlenmesi önerilmektedir.

Abstract

Objectives: Due to the increasing incidence of diabetes and cancer worldwide, there is a need for an updated, comprehensive and accessible summary of the data obtained from studies in this field. This study aimed to update and summarize a systematic review of the literature evaluating the relationship between diabetes and cancer risk.

Material and methods: In this study, the “Umbrella Review” design, designed for the evidence synthesis of existing systematic reviews and meta-analyses prepared by the Joanna Briggs Institute (Joanna Briggs Institution), was used. A quantitative report was made by compiling the summary of the study evidence in line with the umbrella review methodology, examining the prevalence rates (percentage, ratio, relative risk-RR) and heterogeneity rates.

Results: In the studies included in the umbrella review, it was shown that there is a relationship between diabetes and cancer risk. In the methodological quality evaluation, it was determined that all included studies were structured in accordance with a directive, but the research designs were not created on the basis of PICO.

Conclusion: As a result, it was determined that diabetes is an important risk factor for the development of colorectal, pancreatic, endometrial, and ovarian cancer. In this umbrella review, it is recommended that further research be conducted in different populations and different types of cancer in relation to diabetes, followed by standard procedures to synthesize and summarize the evidence.
